The Nurse Clinician II will work within the Behavioral Inpatient Unit of the Hospital. It is an acute, short term, "voluntary" inpatient unit attending to patients ages 13 and older with mental health and/or substance abuse issues.
The Nurse Clinician II provides comprehensive and collaborative nursing care for identified patients from pre-admission t post-discharge, utilizing a multidisciplinary approach.
Identifies and collects data concerning the health status of the patient at the time of admissions and as an ongoing process.
Assesses patients and identifies patient care needs and abnormal findings of patient.
Implements and evaluates a plan of care appropriate for age-specific population.
Formulates nursing orders and implements the action delineated.
Determined nursing diagnoses and formulates a care plan with expected outcomes.
Documents and communicates nursing actions.
Provides for patient/family teaching and discharge planning.
Participates in concurrent and retrospective quality assurance activities that focus on patient care and nursing practice.
Assures safe patient care.
