In order to ensure maximum quality patient care, the Refugee Health Promoter works to improve the health literacy of refugees by coordinating and conducting health orientation and educational workshops for refugee groups, both in the clinic and in the community as well as conducting one-one educational sessions with refugees on such issues as health systems access, dental care, women’s health, chronic conditions, and/or pertinent public health issues. The person selected for this position will work to reduce gaps in health services after initial refugee screening:

  • Coordinate and schedule connection to medical homes and specialty referrals and community services.

  • Lead and organize non-clinical interventions for refugees with most complex needs

  • Design and provide training on refugee resettlement and refugee health to mainstream service providers

This is a grant funded position, grant funded positions are generally expected to end with the completion of the specific grant, unless other funding is acquired. If hired for a grant funded position the supervisor will discuss funding status with the employee prior to the grant’s end.

Education or Formal Training:

  • B.S. or B.A. degree in Human Services, Health, or Education preferred. .

  • Direct care coordination, navigation, or educational experience may be substituted on a year-for-year basis for the bachelor’s degree.

Experience:

  • Previous experience working in social work, behavioral health or community health required.

  • Background in care coordination, health education, or community resources navigation required.

  • Experience working with refugee populations preferred.

  • Bilingual with a preference for a refugee language
