This is a Part-time Weekend Social Worker opportunity located at our downtown Atlanta Medical Center location, a Level I Trauma Center, a 460-bed, Level I Trauma Center and teaching facility which offers a very fast paced work environment with a wide variety of patient diagnoses. The schedule is Friday, Saturday and Sunday 8:00AM-4:30PM.

Responsibilities

The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination. The individual in this position has overall responsibility to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge and prevent avoidable readmission. This position integrates national standard for case management scope of services.

* This position is Friday, Saturday and Sunday - candidates must be able to work weekend days.*

Qualifications

Required Minimum Education, Licensure/Certification and Experience for this position:

  • Master’s degree in Social Work, Psychology or related field.

  • Required Minimum Certification: Current licensure as LMSW or LCSW in the State of Georgia.

  • American Heart Association BLS

  • At least two years related acute hospital patient care experience is required.
