  • PURPOSE OF POSITIONThe primary purpose of the position is to assist in the direction and oversight of all operational and regulatory activities of pharmaceutical services in Home Infusion. The Lead Pharmacist assists in the establishment of standards for quality, productivity and performance in accordance with hospital policies and procedures and requirements of state of Ohio and federal regulatory agencies. RESPONSIBILITIES & EXPECTATIONS• Assists in the planning, supervision, coordination and successful execution of all activities related to the pharmacy's operations.• Assist the Director to develop and maintain pharmacy services in accordance with state and federal regulations, accepted standards, professional practices, hospital and infusion polices. Maintains up to date knowledge of ongoing regulatory changes.• Abides by and promotes agency policies and procedures, regulatory guidelines of CMS, ODH, Ohio Pharmacy Board and the Joint Commission.• Contributes to the supervision of department personnel including: selection, orientation, delegating assessments, training, monitoring, scheduling, evaluation of performance and disciplinary actions.• Assists in the review and revision of the policy and procedures manual as needed.• Participates in the agency's performance improvement plan by assisting in the development, initiation, and ongoing monitoring of the quality improvement plan.• Able to prepare and dispense medications, chemical and pharmaceutical preparation according to written order by authorized medical practitioners with clinical privileges.• Ensures patient safety by managing medication use processes including dispensing administration and monitoring of patient outcomes.• Assist in the development and promotion of collaborative relationship between pharmacy, medical staff, nursing stall and other allied health professionals.• Adequately communicates information to infusion staff and other departments.• Works closely with medical, nursing and pharmacy staff to provide pharmaceutical care to Aultman Infusion patients and to improve patient outcomes at the lowest possible cost.• Maintains clinical expertise and acts as a resource for other staff members in matters of policies, procedures, regulations, and payor coverage issues.• Demonstrates flexibility in meeting the needs of the department by working as an Infusion pharmacist when needed due to staffing requirements or census and responds positively to changes in workflow.• Works with the finance department to ensure claims can be billed properly and accurately.• Provides patient and caregiver education utilizing the teach-back method.• Completes assigned responsibilities in required time frame and accepts additional duties and responsibilities based on the needs of the patient/employee team and that of the agency• Utilizes all applicable computerized system within the department. • Communicates requests, suggestions, concerns or problems to the appropriate personnel.• Completes annual competency requirements.• Complies with agency infection control and safety policies.• Maintains a professional appearance as defined by policy.• Maintains Aultman standards of confidentiality, including the HIPAA Privacy Rule.• Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation.• Supports organizational strategic goals.• Utilize the HRO Tools and Tones to promote patient safety.• Abides by the Aultman Code of Conduct at all times. QUALIFICATIONS· Master's Degree in Pharmacy or Doctor of Pharmacy degree (Pharm.D.) from a college or a university accredited by the American Association of College of Pharmacy preferred.· Has a current license as a Registered Pharmacist in the State of Ohio.· Two years of experience in Infusion pharmacy management preferred but not required· Strong leadership and HRO skills.· Possesses knowledge in project management, performance improvement, risk management and patient safety.· Demonstrates ability to effectively participate in interdisciplinary teams and committees.· Able to work independently and accurately with attention to detail· Excellent interpersonal and oral/written/telephone communication skills; strong organizational and problem-solving skills; ability to stimulate new ideas· Demonstrate emotional intelligence, analytical and critical thinking skills, and problem solving skills· Organized with excellent time management skills, able to meet deadlines· Skilled and complete documentation· Basic computer skills and typing skills· Current BLS certification· Valid Ohio Driver's License· Must have own transportation and provide proof of current liability insurance WORKING CONDITIONS:· Hours of operation (M-F 8 am - 4:30 pm) with shifts as assigned including occasional overtime or scheduled as necessary.
