The Home Health Team Leader is responsible for coordinating a team of staff to assure the continuity of high quality care to home health patients assigned to the team's area. This includes ensuring all team staff are skilled and informed and that they are committed to providing quality care in an efficient manner in accordance with physician's orders and State/Medicare guidelines.


Essential Functions

  • Service - Receives referrals and makes or schedules initial patient evaluation visits as needed.

  • Service - Coordinates the admission of insurance patients.

  • Quality - Initiates and coordinates the patient's plan of treatment through the contacting of community services, therapists, and/or social workers, and through staff assignment to provide patient care as ordered by the physician.

  • Quality - Attends case conference meetings, and takes report on patient's progress with agency personnel to facilitate coordination of care.

  • Service - Coordinates transfer and discharge of patient from agency services as indicated by the physician.

  • Quality - Conducts a minimum of one chart audit per month and reports these findings to the Director of Nurses, Branch Manager, or appropriate agency personnel.

  • Quality - Reviews and provides supervision for each patient's plan of care, through review of services, diagnoses, medications, procedures, progress, clinical course, and goals.

  • Finance - Accountable to Branch Manager for financial and regulatory issues affecting patient care team.

  • People - Assists in the orientation of new agency personnel.

  • Quality - Assists the Director of Nurses and Branch Manager in planning, implementation and evaluation of in-service and continuing education programs for staff.

  • People - Schedules nursing and aide visits and makes necessary changes as needed. Schedules aide care supervision visits for each patient at least every 14 days.

  • People - Assures revision and updates home health aide assignment sheet at least every 60 days and as needed.

  • Quality - Participates in Quality Assurance, Utilization Review, and the overall Performance Improvement plan, program, and process.

  • Growth - Assists in formulation, revision, implementation, and evaluation of policies and procedures, and overall strategic agency planning.

  • Quality - Assures compliance with and ensures timely follow up on daily external vendor clinical edits, outcome support, and coding edits.

  • Quality - Utilizes best practice interventions when finalizing Plan of Care for high risk patients.

  • Service - Work SVP tool to assist with managing of patients daily.

  • Service - Follow up with On-Call events daily.

  • Service - Visits patient according to plan of treatment, completes a progress note for each visit, and submits progress notes to the agency daily.

  • Service - Makes the initial nursing evaluation visit and develops the plan of care within state specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care to supervising nurse within 24 hours of assessment completion.

  • Quality - Participates in the in-service program according to LHC policy.

  • Quality - Performs 60 day record review (recertifications).

  • Quality - Participates in the QA/QI plan and process.

  • Service - Provides services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.

  • Service - Continually evaluates the patient's nursing needs. Instructs and supervises the patient's family in the care of the patient and maintenance of a healthy environment for the patient.

  • Service - Renders nursing care to patients according to the plan of care.

  • Service - Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition.

  • Service - Adheres to and participates in the Care Management process.

  • Quality - Participates in staff conference regarding patient care.


License Requirements

Current RN licensure in Kentucky. Current Driver''s License and vehicle insurance, and access to a dependable vehicle. Current CPR certification required. Home Health RN experience strongly preferred.
